Reporting Incidents:
- Navigation
- Go to the incident reporting module by using the sidebar.
- Click on the Report an Incident to get started
- The Initial Report
For the first part of an Incident Report, you will detail an overview of what happened. This is before filling in specifics for each event that occurred.- About the Incident - Here you will fill in the time, location and associated incident centre for the incident.
- Incident Type - Here you will select the event that occurred in the incident, this can be modified later in the process. If a report contains a near-miss event, it cannot have any other event types.
- Risk - If your reporting configuration allows, you can choose the severity of the incident by selecting an option from the dropdown menus.
- About the Incident - Here you will fill in the time, location and associated incident centre for the incident.
- Filling out an Event
- Person Details - This section requires you to enter the details of the person affected by the incident. You can either fill out the form or select the toggle and use an AssessNet account's information.
- Reporter Details - This section is identical to the person's details but for the incident's reporter.
- About the Event - This will depend on the type of event, in this case, an injury.
- Understanding the Summary Dashboard
Once the first event has been completed, you will be taken to the dashboard for the Incident. This is where you can go into each event and modify/view its contents, as well as add events to be completed.- All categories of events applicable to the incident are shown, with a clear separation between Complete and requiring attention.
- The left sidebar pops out once you hover over it, here you can view each event category as well as the history and adding events.
- Refresh/Summary
This will take you back to the summary page showing all event categories. If you are already on the summary page, this will refresh to show any updates your browser might have missed. - Additional Questions
This event is a list of questions to be filled out, its contents are entirely dependent on what your Organisation's Health and Safety team has set. - Injuries/Other Incident types
You can navigate to all incident / near-miss events by clicking on the associated tab. - Witnesses
This is an event type for documenting witness accounts of events, it will start with their details, then a written account of the incident. - Lost Time
This is a form for documenting lost time due to a person's injuries. - Root Causes
- Here you can document the series of causes for events to occur, eventually determining the root cause of an event. You can have multiple chains of causes.
- You can attach actions to events determined to be a root cause, this will cause the root cause event to only complete once the task has been actioned.
- Attachments
This lets you upload images and documents supporting the incident report, as well as attach AssessNet assessments that relate to the incident report. - Cost Analysis Report
Add the financial damage caused by the incident to a table. - Investigation
Note any findings from investigating the scenario to help determine the root cause if not already found. If any investigation is not completed, the incident cannot be completed and signed off. - Notes
This is where you will add any notes to an incident. - Remedial actions
Add an action to be completed to prevent the incident from reoccurring. - History
Shows changes made to record
